About This Assessment

This tool integrates items adapted from the WHO Adult ADHD Self-Report Scale (ASRS v1.1), executive function indicators from the Brown ADHD Symptom Assessment Scale (BADDS), and functional impairment domains consistent with DSM-5 criteria. It is designed as a structured first-step screening — not a diagnostic instrument.

Think about your behavior and functioning over the past 6 months as you respond. Answer as honestly as possible; there are no right or wrong answers.

⚑ This assessment takes approximately 10–12 minutes to complete. All responses remain private to you and your provider.
Client Information
1

Inattention & Focus

Difficulty sustaining attention, organizing tasks, and following through — core inattentive symptoms

Response Scale: 0 = Never | 1 = Rarely | 2 = Sometimes | 3 = Often | 4 = Very Often
2

Hyperactivity & Impulsivity

Restlessness, impulsive actions, and difficulty regulating activity level

3

Executive Function & Self-Regulation

Working memory, emotional regulation, time management, and activation difficulties — often the most impairing cluster in adults

4

Life Domain Impairment

Check all areas where attention and self-regulation difficulties have caused significant problems in the past 6 months

In which of the following life areas have these difficulties caused you significant problems or interference?
5

Onset & Developmental History

DSM-5 requires symptom onset before age 12 for an ADHD diagnosis — this section captures that context

Please answer all questions before submitting. Your responses will be scored and interpreted automatically.

VisionLogic Clinical Assessment Suite

Clinical Note: This screening tool draws from validated instruments (ASRS v1.1, BADDS, DSM-5 criteria) but is not a diagnostic instrument. Scores above clinical thresholds indicate the need for comprehensive evaluation, including structured clinical interview (ACDS or CAADID), multi-informant ratings (Conners 4), neuropsychological testing, and medical rule-out. Comorbid conditions (anxiety, depression, learning disabilities, sleep disorders, trauma) can produce ADHD-like presentations and must be assessed. A formal diagnosis should be made only by a qualified licensed clinician.
